Abstract

|
We recently experienced a case of thrombotic thrombocytopenic purpura in
a
31-year-old man with microangiopathic hemolytic anemia, thrombocytopenia with easy
friability, fluctuating neurologic symptoms and fever. He did not showed acute renal
failure, one of pentad of thrombotic thrombocytopenic purpura. Severe fragmented red
cells and schistocytoses were present in peripheral blood film. By light microscopic
examination of gingival tissue, the capillary of dermis showed focal obliteration by
fibrinous materials and marked platelet aggregation and deposition of fibrin were
detected on electron microscopy. The plasma of patient collected during plasmapheresis
induced in vitro aggregation of washed platelets from normal donor and these platelet
aggregation was inhibited by normal plasma. He was treated aggressively with plasma
infusion, plasmapheresis, platelet inhibitors, steroid, and immunosuppressive drug, Clinical
and laboratory abnormalities were improved within 14th hospital day.
